Here are the steps on how to start receiving employment services through your waiver:

  • Speak with your case manager. Your case manager is the person who is responsible for coordinating your care and services. They can help you understand your waiver and the employment services that are available to you.
  • Express your interest in exploring employment or working. Let your case manager know that you are interested in working and that you would like to receive employment services.
  • Complete an assessment. Your case manager will complete an assessment to determine your needs and goals. This assessment will help them determine the best way to help you achieve your employment goals.
  • Develop a service plan. Once your assessment is complete, your case manager will develop a service plan with you. This plan will outline the services that you will receive and how they will help you achieve your employment goals.
  • Receive services. Once your service plan is in place, you will begin receiving the services that you need. These services may include job coaching, career counseling, job development, and other forms of support.
